Community-based, multi-disciplinary teams provides services to chronically mentally ill adults including medication monitoring, assessment, counseling, advocacy, and other ongoing support, delivered off-site. For severe and persistent mental illness w/multiple hospitalizations, treatment-resistant, failure to benefit from 6 months treatment.
